Understanding Your Coverage:

It is important to understand your insurance coverage adequately as you are responsible for appointment cost if your insurance does not cover the nutrition session:

(1) You will be required to submit a physician referral form. As a courtesy we reach out to your physician office for a referral. Please note a referral does not automatically ensure insurance coverage.

(2) You will be required verify your insurance coverage for nutrition counseling services. As a courtesy, we will contact your insurance provider to verify your benefits and provide you with details such as the number of covered sessions, co-pay, co-insurance, and deductibles. However, if you would like to verify coverage yourself or have any questions regarding your insurance coverage, please contact your insurance provider directly.

(3) The cost of a nutrition counseling session is $179 if it’s not covered by insurance
