Please use the form below to request your FIRST nutrition appointment utilizing Medicare insurance coverage. Vanessa Avila, MS, RD is in-network with Original Medicare, Aetna Medicare Advantage plans, and United HealthCare Medicare Advantage plans.

Before you schedule your appointment, please ensure we’ve received all required forms: a physician referral, verification of benefits, and copies of your Medicare insurance card, as well as a valid driver’s license or state ID. These documents are crucial for confirming your appointment. If any are missing, we won’t be able to proceed, and your appointment will have to be canceled.
